Directions for completing the Entry Form: 1.
Fill in personal information at the top: Name, current date, address, telephone
number, email address and whether this exhibitor is entering as a junior or
senior. All entries listed must be for
the exhibitor listed on this form. Use separate forms for each exhibitor.
This form should NOT be used to register any livestock. The appropriate
forms for livestock can be found in the Premium Book or on the Premium Book web page
and should be sent to the proper department chairpersons. 2.
Find the correct Department, Section, and Class for your item, using the
Premium Book. 3.
Fill in the class description listed in the Premium Book. This is very important as it is used to
verify that the section and class are input correctly. 4.
If you have any questions about filling out your entry form, please call the
appropriate department chairperson listed in the Premium Book. 5.
